What Is ESDM Therapy and Does OAP Cover It?

Direct answer

The Early Start Denver Model (ESDM) is an evidence-based, play-based intervention for autistic children aged 12-48 months that combines ABA principles with developmental and relationship-based approaches. Research by Dawson et al. (2010) in Pediatrics demonstrated significant improvements in IQ, adaptive behaviour, and diagnostic status. OAP core clinical funding covers ABA, and ESDM uses ABA teaching strategies. The OAP guidelines do not mention ESDM certification, but the ABA must be delivered or supervised by a behaviour analyst, psychologist or psychological associate registered with the College of Psychologists and Behaviour Analysts of Ontario.

Why Timing Matters

The wait outlasts the window.

Clinical research (American Academy of Pediatrics; Hyman et al., Pediatrics 2020) identifies birth to age six as the peak neuroplasticity window for autism intervention. Ontario's reported minimum wait is 5+ years for families registered in August 2021 (community-reported, Ontario Autism Coalition, July 2026).

Registered at age 3. Services begin at age 8, after 5+ years for families registered in August 2021 (community-reported, Ontario Autism Coalition, July 2026), most of it inside the intervention window.

✓ Intervention before 6

Research shows 2× greater gains in cognitive and adaptive functioning. Some children lose their diagnosis entirely. The brain's plasticity makes early therapy dramatically more effective.

◌ Intervention after 6

Reduced neuroplasticity means slower progress, higher lifetime costs, and poorer outcomes. Every year of delay narrows the range of achievable milestones. Ontario's waitlist guarantees this for most families.

Of the 71,263 children on the unfunded backlog, the majority were registered between ages 2 and 4. At the current pace, most will age past 6 before receiving any core funding , missing the window that the American Academy of Pediatrics and every major autism research body identifies as critical.

How ESDM Works

  • ESDM integrates ABA teaching strategies with developmental and relationship-based approaches. Therapy is delivered in naturalistic settings (play, daily routines) rather than structured table-top drills. A certified ESDM therapist follows a curriculum checklist covering receptive communication, expressive communication, social skills, imitation, cognition, play, fine/gross motor, and self-care domains.
  • The landmark Dawson et al. (2010) randomized controlled trial published in Pediatrics found that children receiving ESDM for two years (20 therapist hours a week were offered; children averaged about 15) showed significant improvements in IQ (average 17.6-point gain), adaptive behaviour, and autism diagnosis severity compared to community treatment controls.

ESDM and the Ontario Autism Program

  • OAP core clinical funding covers applied behaviour analysis (ABA), and ESDM uses ABA teaching strategies. The Ontario guidelines do not name ESDM, so check with your AccessOAP care coordinator if you are not sure it is eligible. Families invited to core clinical services can use their OAP funding allocation for ABA, which may include ESDM, from a provider who meets the Ontario qualification rules. The OAP guidelines do not mention ESDM certification, and joining the OAP provider list is voluntary for clinicians.
  • For families waiting for core services, the OAP offers free caregiver-mediated early years programs for children 12 to 48 months old. ESDM is not one of the six programs offered, but some, like Project ImPACT and Social ABCs, also mix developmental and behavioural strategies. For OAP core clinical services, Ontario guidelines say fees should be agreed on by the family and provider before services start.

Frequently asked questions

It can be. OAP core clinical funding covers applied behaviour analysis (ABA), and ESDM uses ABA teaching strategies. ESDM was designed for children aged 12-48 months (1-4 years). Some providers offer adaptations for slightly older children, but the strongest evidence base is for toddlers.

ESDM uses ABA principles but delivers them in play-based, naturalistic interactions rather than structured discrete trial training. It emphasizes the child-therapist relationship, joint attention, and following the child's motivation within everyday activities.
