How long does autism diagnosis take in Ontario?

Before joining the OAP waitlist, Ontario diagnostic waitlists average **12–24 months** at public hospitals. [OAP] This pre-waitlist delay means total time from first concern to therapy often exceeds **5–7 years**, an invisible bottleneck in official statistics.

Source: Ontario Autism Program [OAP]

Is private autism assessment faster in Ontario?

Private autism assessments cost **$2,000–$4,000** but reduce wait times from years to weeks. [OAP] Many families face the choice of paying out-of-pocket to access the OAP sooner or waiting while their child misses the critical early intervention window.

Source: Ontario Autism Program [OAP]

What are the out-of-pocket costs for autism therapy?

Private ABA therapy costs $100-$150/hour (BCBA supervised). Intensive programs of 20+ hours/week can cost up to $95,000 USD/year (2020 US cost estimate cited in FAO 2020 report; Canadian costs vary). Speech and OT cost $120-$160/hour. These costs are often prohibitive while families wait 5+ years for OAP funding.

SYSTEM OVERVIEW

How Ontario's Autism System Works

The Ontario Autism Program (OAP) is designed to provide funding for therapy, but understanding the eligibility and wait times is critical.

Program Summary

The Ontario Autism Program (OAP) provides funding to families of children and youth on the autism spectrum. Families must register with AccessOAPto enter the system. Once registered, children are placed on a waitlist for "Core Clinical Services" funding.

Age-Based Funding

Eligible families receive funding based on the child's age: $22,000/year (ages 0-5) or $5,500/year (ages 6-17).

Wait Times

Current wait times for Core Clinical Services funding are estimated at 5+ years for new registrants.

ASSESSMENT PROCESS

What's Included in an Autism Assessment

A comprehensive autism assessment in Ontario follows standardized protocols and includes multiple components to ensure diagnostic accuracy.

1

Clinical Interview

Detailed discussion of developmental history, current concerns, medical history, and family history. For children, this involves extensive parent interview.

2

Direct Observation

The assessor observes and interacts with the individual. For children, this is play-based. For adults, this involves structured conversation and social tasks.

3

Standardized Tools

Gold-standard assessments like ADOS-2 (Autism Diagnostic Observation Schedule) and ADI-R (Autism Diagnostic Interview-Revised) provide objective evidence.

4

Cognitive Testing

IQ or developmental testing to establish cognitive baseline and identify strengths and challenges. This helps differentiate autism from other conditions.

5

Adaptive Assessment

Evaluation of daily living skills, communication, and social functioning in everyday contexts. This helps determine support needs.

6

Report & Feedback

Comprehensive written report with diagnosis, recommendations, and feedback session to discuss findings and next steps.

Typical Time Investment

Child Assessment

6-10 hours total, spread over 2-3 sessions

Longer because it involves parent interview, child observation, and more extensive testing

Adult Assessment

4-8 hours total, spread over 1-3 sessions

May be slightly shorter but requires more specialized expertise

How to Get Started

Schedule an appointment with your family doctor or pediatrician. Be specific about your concerns. Bring a list of observed behaviors and symptoms that suggest autism.

Common signs to mention:

  • • Delayed speech or unusual language patterns
  • • Difficulty with social interaction or making friends
  • • Repetitive behaviors or intense interests
  • • Sensory sensitivities or aversions
  • • Regression in developmental milestones

Request a referral to an autism assessment center. For children, ask for referral to your local hospital's autism assessment program or developmental pediatrician.

Tip: If your doctor is unfamiliar with autism assessment, consider requesting a referral to a developmental pediatrician or seeking a second opinion. Be persistent if you have genuine concerns.

After your referral is received, you will be placed on a waitlist. Current OHIP wait times are 18-24 months. If this is too long, consider private assessment options.

OHIP Assessment

Free, 18-24 month wait

Private Assessment

$2,500-$4,000, 3-8 week wait

Attend all scheduled assessment appointments. The process typically involves 2-3 sessions totaling 6-10 hours for children, or 1-3 sessions for adults.

What to bring: Previous assessments, school reports, medical records, and any relevant documentation. For children, bring information about developmental milestones and behaviors at home and school.

After the assessment, you will receive a detailed report with the diagnosis. If the diagnosis is autism spectrum disorder, register immediately with AccessOAP.

Frequently Asked Questions

Autism diagnosis costs in Ontario range from free (OHIP-covered) to $2,500-$4,000 for private assessments. Public assessments at hospitals are covered by OHIP but have 18-24 month wait times. Private assessments cost $2,500-$4,000 for children and $3,000-$5,000 for adults, with shorter wait times of 3-8 weeks. Both options are accepted for OAP funding if conducted by qualified professionals.

Yes, autism assessments are covered by OHIP in Ontario when conducted at publicly-funded hospitals and assessment centers including Holland Bloorview, SickKids, CHEO, and similar facilities across the province. However, wait times for OHIP-covered assessments are 18-24 months. You need a referral from your family doctor or pediatrician to access OHIP-covered assessment.

Public (OHIP-covered) assessments have wait times of 18-24 months at major centers. Some regional centers may have shorter waits of 12-18 months, while urban centers face longer waits. Private assessments have much shorter wait times of 3-8 weeks, though costs range from $2,500-$5,000. The wait time clock starts when your referral is received.

Yes, an official autism diagnosis is required to qualify for the Ontario Autism Program (OAP) funding. The diagnosis must be made by a qualified professional: registered psychologist, psychological associate, developmental pediatrician, pediatrician, or psychiatrist. Both public and private assessments are accepted for OAP eligibility. Once diagnosed, register immediately with AccessOAP by calling 1-833-425-2445 to secure your place in the funding waitlist.

Consider private assessment if: your child is 2-6 (early intervention critical), you can afford $2,500-$4,000, you want to access OAP funding 15+ months sooner, or you need diagnosis urgently for school accommodations. Wait for OHIP if: you cannot afford private costs, your child is over 6 (less time-sensitive), you're willing to wait 18-24 months, or you can access interim supports through schools and community resources.

The ADOS-2 (Autism Diagnostic Observation Schedule) is the gold standard tool for autism diagnosis. It involves direct observation of social interaction, communication, and play. While not absolutely required, most comprehensive assessments include ADOS-2 because it provides objective, standardized evidence of autism traits. It is often combined with ADI-R (parent interview about developmental history).

Adult autism diagnosis in Ontario typically costs $3,000-$5,000 for private assessments, compared to $2,500-$4,000 for children. Adults pay more because assessments take longer and require more specialized expertise. OHIP-covered adult assessments are available but wait times are often 24+ months, and fewer providers offer adult assessments. Adults don't qualify for OAP funding but diagnosis unlocks workplace accommodations and DTC eligibility.
