Before you call AccessOAP: Gather these documents to ensure smooth registration. Missing documents can delay your registration date—which determines your waitlist position.

Remember: Your child's waitlist position is based on the date you complete registration, not the diagnosis date.

Essential Documents Required

Autism Diagnosis Report Required

Must be on letterhead, signed by a qualified professional (psychologist, psychological associate, or physician), include registration number, and clearly state ASD diagnosis.

Proof of Ontario Residency Required

Ontario driver's license, photo card, or utility bill with your name and Ontario address.

Child's Ontario Health Card Required

Valid health card number and version code. Have the physical card ready when calling.

Proof of Canadian Citizenship/Status Required

Birth certificate, Canadian passport, permanent resident card, or other immigration document.

Guardianship Documentation (if applicable) If Applicable

Legal custody, guardianship, or Children's Aid Society documentation if you are not the biological parent.

Supporting Documents Recommended

Assessment Reports Have Ready

ADOS-2, ADI-R, or other standardized assessment reports. These support the diagnosis and help determine needs level.

School IEP or Accommodations Have Ready

Current Individual Education Plan or any school accommodations already in place.

Previous Therapy Reports Have Ready

Speech therapy, occupational therapy, or ABA reports if your child has already received services.

Medical Referral Letters Have Ready

Referral letters from pediatrician, family doctor, or other specialists.

Contact Information

Your phone number, email address, and mailing address. Alternate contact person recommended.

Child's Information

Full legal name, date of birth, current school or daycare, and any special needs or safety concerns.
